Komatsu offers two medical plan options to choose from. Each plan is paired with a health account that includes wellness-based contributions from the company to help you pay for eligible medical and prescription expenses. Since these accounts work differently, it’s important to understand how each one functions.
PPO Plan: A traditional PPO plan paired with a Health Reimbursement Account (HRA).
HSA Plan: A health plan paired with a Health Savings Account (HSA).
Both plan options are administered by Blue Cross Blue Shield of IL and use the same network of providers. You can search for network providers at bcbsil.com.

How the account works — As you use medical services and fill prescriptions, the plan automatically applies your company-provided HRA dollars toward expenses until the account is depleted. If you use all of the money in your account, you will pay any additional expenses (deductible, coinsurance or copays) out of your pocket. Contributions* — If you qualify for the wellness reward, Komatsu contributes to your HRA each year based on your coverage level ($200 for single, $400 for family). See page 8 for wellness reward details. You cannot make contributions to the account. However, unused HRA dollars roll to the next year, as long as you remain in the PPO Plan. There is no limit on how high your HRA balance can grow. If you change medical plans or leave the company, unused money in your account will be forfeited. | How the account works — You decide if you want to use your HSA dollars toward eligible health care expenses or save the money for future needs. Money goes into the HSA tax-free, grows tax-free with interest or earnings, and remains tax-free when used on eligible expenses. You can easily access your account dollars with the HSA debit card. Contributions** — Komatsu contributes to your HSA each year based on your coverage level ($300 for single, $600 for family). Flexible Spending Accounts (FSA): Tax savings for health care and daycare expenses
An FSA is an easy way to set aside pretax money to pay for health care or dependent care expenses. You choose how much you want to contribute each pay period (before taxes). Plan your contributions carefully — unused funds that exceed the carryover limit are forfeited, although you do have until March 31 of the next year to submit expenses for reimbursement.
Health care FSA is available to any employee enrolled in the PPO.
The health care FSA reimburses you for eligible medical, prescription, dental and vision care expenses, including deductibles and copays.
Expenses for all covered family members are considered eligible.
You must re-enroll every year to participate.
You can use the health care debit card to pay for eligible expenses at point of purchase or submit claims for reimbursement.
Annual contribution limits may change year to year.
Only a certain amount of unused funds can be carried over into the next year.
FSA for dependent care and elder care
The dependent care FSA covers expenses, such as day camp and daycare, for children up to age 13. You may also use your dependent care FSA to pay for elder care expenses. This FSA does not cover health care related expenses.
With the dependent care FSA, you have access to your elected amount only after the money has been deposited into your account. You can contribute up to $7,500 per year pretax, but some or all of that amount may be taxable to you when you file your taxes.
If you’re married and file taxes separately, amounts over $3,750 are taxable.
If your spouse’s income is less than your contribution, amounts in excess of their income are taxable.
There is no carryover limit for a dependent care FSA, unused funds are forfeited at the end of the year.

Our Core Plan is best for individuals or families who like to stay on top of regular cleanings, expect minimal dental work and want to keep monthly costs down.
