Completing and submitting the Credit Application authorizes Komatsu Financial to obtain information from any credit reporting agency and authorizes the named banks and financial institutions to release such information as necessary to establish credit with Komatsu Financial.

To apply for credit approval:

Signing up for AutoPay is quick, easy and economical.

By completing the AutoPay form and submission to Komatsu Financial, you authorize Komatsu Financial Limited Partnership to initiate electronic deduction from the bank account you designate.

To enroll in AutoPay:

• Read the AutoPay Agreement thoroughly
• Complete all requested information
• Print and sign the AutoPay agreement

Monitor your account to verify that the first payment is withdrawn. If you change financial institutions or accounts, contact Komatsu Financial and submit a new AutoPay application. Be sure to allow at least seven business days before the payment due date for the change to be processed.
