PPP request

To request the PPP (Professional Profile Form), please send the following documentation to the email address below: solicitacao.ppp@komatsu.com.br

1. ID and CPF
2. PIS or Citizen Card
3. Employment record book (pages relating to Komatsu records):
   • Photo
   • Civil qualification
   • Employment contract (all, if there is more than one from Komatsu)
   • Salary changes (all relevant to Komatsu)
   • General notes (all relevant to Komatsu)

Important: The request must be made by the former employee themselves. If the transaction is being carried out by another person or lawyer, you must send the power of attorney stating that they have been granted powers to represent you.
